Comorbid illness in women with chronic fatigue syndrome: a test of the single syndrome hypothesis.

This study looked at 163 women with ME/CFS to see whether having other conditions like fibromyalgia or chemical sensitivities alongside ME/CFS made their illness different. About a third of the women had fibromyalgia, and another third had chemical sensitivities. The main finding was that women with multiple conditions were more likely to have depression than those with ME/CFS alone, but otherwise their symptoms and disability levels were fairly similar.

This study looked at how different definitions of unexplained fatigue affect how many people are diagnosed and whether they also have depression or anxiety. Researchers interviewed over 5,400 primary care patients from 14 countries and found that when they required more physical symptoms in addition to fatigue, fewer people were diagnosed, but those diagnosed were more likely to also have depression or anxiety.

Comparison of subjective and objective measures of insomnia in monozygotic twins discordant for chronic fatigue syndrome.

People with ME/CFS report feeling like they sleep poorly and don't rest well, but when researchers measured their actual sleep using medical equipment, the sleep looked similar to their healthy identical twins. The main difference was that people with ME/CFS had more REM sleep (a specific sleep stage) and felt much less refreshed after sleeping, even though the equipment showed they were sleeping normally. This suggests that ME/CFS may involve a mismatch between how sleep actually is and how it feels.

Life events, difficulties and dilemmas in the onset of chronic fatigue syndrome: a case-control study.

This study compared 64 people with ME/CFS to 64 similar people without the condition to see if stressful life events played a role in developing the illness. Researchers found that ME/CFS patients were much more likely to have experienced severe stress or difficult situations in the few months before getting sick. Particularly interesting was that about 30% of ME/CFS patients had faced an unsolvable dilemma (a 'no-win' situation) before illness onset, while none of the healthy controls had experienced this.

This study looked at how hard exercise feels to people with ME/CFS compared to healthy people. Researchers had both groups exercise on a treadmill and rate their effort. While people with ME/CFS said exercise felt harder at moderate intensities, when researchers compared effort levels to each person's maximum oxygen capacity, there was no real difference—suggesting the difference in how hard it felt might be explained by their lower fitness level rather than a problem with how their body perceives effort.

This study found that ME/CFS patients do experience this adaptation effect, where sleep measurements like total sleep time and sleep quality improve on the second night. Understanding this pattern helps doctors interpret sleep test results more accurately.

They measured heart rate and blood vessel responses using advanced mathematical analysis that detects patterns typically missed by standard methods. People with ME/CFS showed very different heart responses compared to healthy people and those with high blood pressure, suggesting their cardiovascular systems react abnormally to physical stress.

They found that people with ME/CFS had a specific increase in a immune system marker called C4a six hours after exercise, and their fatigue and reduced activity worsened significantly, while healthy controls did not show these changes.

This study found that women with endometriosis—a painful condition where tissue grows outside the uterus—have much higher rates of ME/CFS, fibromyalgia, and autoimmune diseases compared to the general population. Researchers surveyed nearly 3,700 women with endometriosis and compared their health conditions to published data about women without endometriosis. Women with endometriosis were 150 times more likely to have ME/CFS and nearly twice as likely to have fibromyalgia.

This study tested whether people with ME/CFS have actual problems with memory and thinking skills, or whether the difficulty they experience is mainly related to depression, anxiety, or fatigue. Researchers compared 23 people with ME/CFS to 23 healthy people using both objective tests (like memory tasks) and subjective reports (how people felt their thinking was affected). The findings showed that on actual cognitive tests, people with ME/CFS performed just as well as healthy controls, though they reported feeling that their thinking was worse.

The prevalence of chronic fatigue syndrome in Iceland - a national comparison by gender drawing on four different criteria.

This study asked over 2,500 Icelanders about symptoms of chronic fatigue syndrome (CFS) to find out how many people have it. Different diagnosis criteria gave very different results—ranging from 0% to 4.9%—showing that how doctors define CFS matters a lot. The study found that women made up most of the people with CFS, and there were some interesting differences between men and women in their symptoms.

When patients with ME/CFS completed a challenging thinking task, they consistently thought they did worse than they actually did, even though their actual performance matched healthy people's performance. This gap between what they thought they could do and what they actually accomplished was linked to how mentally tired and fatigued the task made them feel.

A comparison of patients with chronic fatigue syndrome attending separate fatigue clinics based in immunology and psychiatry.

This study compared ME/CFS patients attending two different hospital clinics—one run by immunology doctors and one by psychiatry doctors—to see if they were similar. Researchers found that patients at both clinics had very similar symptoms, disability levels, and emotional distress, even though they chose different clinics. The main difference was that immunology clinic patients were more likely to believe their fatigue was caused by physical problems, while psychiatry clinic patients were more open to other explanations.

They found no significant differences in ANA levels between any of the groups, and none of the patients had the specific nuclear envelope ANA type that had been suggested as a possible ME/CFS marker.

This study looked at immune system differences in identical twins where one twin had ME/CFS and the other didn't. By comparing identical twins, researchers could focus on immune changes related to the illness rather than genetic differences. The study found that certain immune cells were more variable in twins with ME/CFS, suggesting the immune system may be less stable in this condition.

This study looked for four types of bacteria called Mycoplasma in the blood of 261 ME/CFS patients in Europe and compared them to 36 healthy people. The researchers found that about 69% of ME/CFS patients had at least one type of Mycoplasma bacteria, compared to only 6% of healthy controls. The most common type found was M. hominis, followed by M. pneumoniae and M. fermentans.

Physical performance and prediction of 2-5A synthetase/RNase L antiviral pathway activity in patients with chronic fatigue syndrome.

This study looked at whether exercise test results could predict high levels of RNase L, an enzyme that may be involved in ME/CFS. Researchers asked 73 patients to exercise until they were exhausted, and found that those with elevated RNase L had lower exercise capacity than those with normal levels. The findings suggest that simple exercise tests might help doctors identify which ME/CFS patients have this particular biological abnormality.
