Relationship Between Chronic Fatigue Syndrome and Neurally Mediated Hypotension.

This study explored a possible connection between ME/CFS and a condition called neurally mediated hypotension, where blood pressure drops suddenly due to problems with nerve signals that control blood vessels. This study followed 250 people who had either glandular fever (caused by Epstein-Barr virus) or a common cold to see how many developed long-lasting fatigue. Nearly half of those with glandular fever experienced severe tiredness immediately after infection, compared to only one-fifth with a common cold. About 9-22% of people with glandular fever developed chronic fatigue syndrome within six months, suggesting that this infection may trigger ME/CFS in some patients.

Chronic fatigue syndrome: identification of distinct subgroups on the basis of allergy and psychologic variables.

This study looked at whether allergies and psychological factors play a role in ME/CFS. Researchers compared 18 people with ME/CFS to healthy controls, people with allergies, and people with depression. They found that people with ME/CFS had different immune molecules (called cytokines) in their blood compared to healthy people, similar to those with allergies. Interestingly, people with ME/CFS who also had certain psychological profiles showed different immune patterns, suggesting that both allergies and how the mind responds to them may contribute to ME/CFS in some patients.

Salivary cortisol profiles in chronic fatigue syndrome.

This study measured cortisol (a stress hormone) levels in the saliva of 10 people with ME/CFS and 10 healthy people over a 16-hour period. This finding suggests that low cortisol is not the cause of fatigue in ME/CFS.

For ME/CFS patients, the key factors were how manageable the disease feels, how serious it is, whether they feel personally responsible, and whether they see external causes as responsible.

Impaired associative learning in chronic fatigue syndrome.

This study tested whether people with ME/CFS have problems with how their brain learns and forms connections between different pieces of information. Researchers measured brain learning using a test where a gentle puff of air is paired with an eye blink stimulus. While ME/CFS patients could hear and feel the stimuli normally, they had difficulty learning to associate the two stimuli together—suggesting their brain's ability to form certain types of connections may be affected.

This study looked at a marker in the blood called neopterin, which may indicate immune system activation, in women with chemical sensitivity (difficulty tolerating everyday chemicals at low levels), women with depression, and healthy women. The researchers found that neopterin levels themselves were similar across groups, but in women with chemical sensitivity, higher neopterin levels were linked with more physical symptoms—a connection not seen in the other groups.

Prevalence of chronic fatigue syndrome in a community population in Japan.

Researchers found that about 1 in 67 people had severe, persistent fatigue that met ME/CFS criteria and significantly disrupted their daily life. The findings suggested ME/CFS might be more common in Japan than in Western countries studied previously.

This study tested whether a comprehensive treatment program combining medical care, psychiatric treatment, and cognitive-behavioral therapy (talk therapy focusing on thoughts and behaviors) could help people with ME/CFS. Fifty-one patients received this treatment, which included supervised exercise, sleep strategies, activity planning, and therapy. The results showed that about 61% returned to work or functioned at a work-like level, while 33 months later, those who completed treatment maintained their improvements, whereas untreated patients rarely improved.

Lack of seasonal variation of symptoms in patients with chronic fatigue syndrome.

Many people experience changes in energy, mood, appetite, and sleep across different seasons, often feeling worse in winter. This study tested whether ME/CFS patients experience these same seasonal changes. The researchers found that ME/CFS patients actually show much less seasonal variation in their symptoms compared to healthy people, suggesting their fatigue and other symptoms stay relatively constant year-round rather than fluctuating with the seasons.

Detection of Mycoplasma genus and Mycoplasma fermentans by PCR in patients with Chronic Fatigue Syndrome.

Researchers tested blood samples from 100 ME/CFS patients and 50 healthy people to see if a bacterium called Mycoplasma fermentans was present. They found this bacterium's genetic material in more than a third of ME/CFS patients compared to only 8% of healthy controls. Patients with higher amounts of the bacterium also had stronger immune responses against it, suggesting their bodies were reacting to the infection.

Researchers found that about half of hearing loss patients had antibodies against phospholipids and about half to two-thirds had antibodies against serotonin and gangliosides. Interestingly, some patients with hearing loss also had symptoms of ME/CFS and fibromyalgia, suggesting these conditions may be connected through shared immune system abnormalities.

Researchers used a special MRI scanner to measure how muscles use energy during exercise by looking at chemical changes in the calf. They found that healthy people used two different types of energy pathways during exercise, while people with ME/CFS showed unusual resting energy levels but normal recovery after exercise—unlike patients with genetic metabolic disorders, whose muscles recovered very slowly.

Researchers compared blood immune system markers between ME/CFS patients, people with depression, people with multiple sclerosis, and healthy controls. They measured 18 different immune system measurements, looking for signs that ME/CFS involves immune system problems. The study found only minor differences in one immune protein between ME/CFS patients and healthy controls, and these differences disappeared when comparing across all three disease groups.

Reproductive correlates of chronic fatigue syndrome.

This study compared reproductive and menstrual histories in women with ME/CFS to healthy women without the condition. Women with ME/CFS reported more gynecologic problems and irregular menstrual cycles, and researchers found signs suggesting ovarian dysfunction—such as polycystic ovary syndrome (PCOS)—occurred more often in the ME/CFS group. The authors propose that hormonal imbalances affecting egg release may contribute to ME/CFS by reducing protective immune effects from certain hormones.

This study found that people with ME/CFS have unusually large swings in heart rate and blood pressure throughout the day and night, even though the overall pattern matches healthy controls. Importantly, their nighttime blood pressure drops too low (below 100 mm Hg systolic), which may contribute to fatigue and other symptoms. When treated with a heart-strengthening medication called inopamil, nighttime blood pressure improved, but melatonin actually made it worse.
