A German research team reviewed 44 published sources to map how long COVID and related conditions—including ME/CFS—are managed within Germany's healthcare system. They report that patients often encounter information gaps, unclear responsibilities, long waiting times, and sometimes receive advice to increase activity despite the risk of post-exertional malaise. The review is preliminary and describes the current situation; it does not evaluate whether specific treatments work or how to fix these gaps.
By analogy, this health systems map may be relevant to ME/CFS patients and clinicians in Germany and similar healthcare contexts. The observed barriers—navigational difficulty, diagnostic delay, risk of harmful activity prescription despite post-exertional malaise—are reported across long COVID and ME/CFS populations, suggesting common gaps in care infrastructure and clinician awareness that warrant attention.
This scoping review does not establish causation, does not quantify the prevalence or severity of care gaps, does not evaluate the efficacy of any intervention or care model, and does not compare outcomes between different healthcare systems. It is a descriptive synthesis of existing literature, not a randomised trial or cohort study with comparators. The generalisability to ME/CFS specifically—as opposed to long COVID broadly—remains unclear.
About the PEM badge: “PEM required” means post-exertional malaise was an explicit required diagnostic criterion for participant inclusion in this study — not that PEM was studied, observed, or discussed. Studies using criteria that do not require PEM (e.g. Fukuda, Oxford) are tagged “PEM not required”. How the atlas works →
