This study looked at how often people with ME/CFS experience two symptoms that doctors use to diagnose the condition: difficulty thinking clearly (brain fog, memory problems) and dizziness when standing up. The researchers found that nearly all participants reported thinking and memory problems, while only a small number had standing-up dizziness without also having cognitive problems. This suggests these two symptoms overlap heavily in ME/CFS patients.

Factors Influencing the Prognosis of Patients with Myalgic Encephalomyelitis/Chronic Fatigue Syndrome.

This study looked at 168 ME/CFS patients to understand what factors help people recover or significantly improve. The research found that only about 13% of patients experienced recovery or substantial improvement over time. Interestingly, patients who were older when they first got sick were more likely to improve, and people who received a diagnosis sooner (without long delays) also had better outcomes. The study confirms that ME/CFS is generally a serious condition with poor prognosis, but suggests that improvement is possible at any age.

Orthostatic Intolerance in Long-Haul COVID after SARS-CoV-2: A Case-Control Comparison with Post-EBV and Insidious-Onset Myalgic Encephalomyelitis/Chronic Fatigue Syndrome Patients.

This study compared patients with long-haul COVID to two groups of ME/CFS patients (those who got sick after Epstein-Barr virus infection and those who became ill gradually) to understand a common symptom called orthostatic intolerance—dizziness or fainting when standing up. Researchers found that long-haul COVID patients had similar symptoms and physical responses to standing as ME/CFS patients, and all three groups showed comparable problems with blood flow and heart function during the standing test. The findings suggest that long-haul COVID and ME/CFS may be essentially the same underlying condition.

Researchers followed 148 people with ME/CFS for 6 months to understand why some patients improve while others don't. They tracked activity patterns, daily stressors and positive experiences, and measured heart rate variability (a sign of nervous system health). Patients who didn't improve had less heart rate variability and reported fewer positive daily experiences, but surprisingly, activity patterns like "push-crash" cycles didn't differ between improving and non-improving groups.

Specifically, the researchers discovered that after COVID-19, dormant viruses (like EBV and others) reactivate more strongly in people with ME/CFS, as shown by antibody patterns in saliva. This suggests that COVID-19 may trigger hidden virus reactivation that could contribute to fatigue symptoms in ME/CFS patients.

These brain differences were also connected to how severe symptoms like fatigue, sleep problems, and cognitive difficulties were in each person.

This study looked at how many people in Australia have ME/CFS by examining medical records from primary care clinics between 2015-2019. Researchers found that roughly 95-104 people per 100,000 have ME/CFS in Australia, but they also discovered that many patients avoid visiting their doctors because of stigma, disbelief in the condition, and lack of available treatments. The researchers concluded that the real number of people with ME/CFS is likely much higher than what their numbers show.

Characteristics of Sleep Disturbance in Patients with Long COVID: A Retrospective Observational Study in Japan.

This study looked at sleep problems in 363 long COVID patients in Japan and found that about 16.5% had significant sleep disturbance. People infected during the Omicron wave were twice as likely to have sleep problems compared to those infected during the Delta wave. Patients with sleep disturbance also reported more fatigue, brain fog, headaches, and anxiety, and had abnormal stress hormone levels suggesting their bodies were under persistent stress.

Generalized Worry in Patients With Chronic Fatigue Syndrome Following Cognitive Behavioral Therapy: A Prospective Cohort Study in Secondary Care.

This study looked at worry in nearly 500 people with ME/CFS who received a talking therapy called cognitive behavioral therapy (CBT). About 7 in 10 patients had significant worry alongside their fatigue. After CBT, patients reported notably less worry, and this improvement stayed stable months later. People who worried severely at the start also experienced worse fatigue and had more difficulty with work and social activities.

How mycobacterium tuberculosis infection could lead to the increasing risks of chronic fatigue syndrome and the potential immunological effects: a population-based retrospective cohort study.

This study found that people who have had a tuberculosis infection are about 23% more likely to develop ME/CFS than people who have never had tuberculosis. Researchers in Taiwan tracked over 38,000 people for about 10 years and noticed that tuberculosis and ME/CFS share similar symptoms and immune system patterns. The link was especially strong in men and in people aged 65 and older.

Researchers found that COVID-19 patients reported more fatigue and post-exertional malaise (PEM)—a worsening of symptoms after physical activity—and had slightly elevated markers of brain injury in their blood. However, their thinking and memory abilities tested normally, and the blood markers didn't directly correlate with their symptoms.

Findings from a survey about hidradenitis suppurativa in the Netherlands.

Hidradenitis suppurativa (HS) is a painful skin condition that causes recurrent boils, drainage, and scarring in areas like armpits and groin. This Dutch study surveyed over 167,000 people and found that HS is more common than previously thought (2% rather than 1%), and that people with HS often have other conditions including chronic fatigue syndrome, fibromyalgia, and depression. The study also found that many people with HS have not yet been diagnosed or treated.

Some people with Long COVID have abnormal immune proteins called autoantibodies that attack blood vessel control systems in their bodies. This study found that patients with these specific autoantibodies had reduced blood flow in the tiny vessels at the back of the eye, which may reflect similar problems throughout their whole body and could explain some of their symptoms.

This survey asked 812 people with long COVID about how COVID-19 vaccination affected their symptoms. Overall, 58% reported feeling better after vaccination, 18% felt worse, and the rest noticed no change. People who received mRNA vaccines (like Pfizer and Moderna) tended to see more improvement than those who received other types of vaccines. However, responses varied greatly from person to person.

This study looked at over 12,000 people in Taiwan with ME/CFS and compared them to an equal number of similar people without the condition. Researchers found that people with ME/CFS are much more likely to also have depression, anxiety, sleep problems, and certain other illnesses like diabetes and rheumatoid arthritis. The study also found that doctors commonly prescribed antidepressants, anti-anxiety medications, pain relievers, and recommended exercise and therapy—treatments that earlier research suggested might help with ME/CFS symptoms.

This study compared 205 people diagnosed with ME/CFS to 57 people with similar symptoms who were not diagnosed with ME/CFS. The researchers found that those with an ME/CFS diagnosis reported greater difficulties with physical and mental effort, more pain, and worse overall health than the undiagnosed group. Interestingly, both groups reported similar levels of anxiety, depression, and weekly physical activity, suggesting that what makes ME/CFS distinct may be specifically how the body reacts to exertion rather than activity levels alone.

This study examined whether patients with long-lasting neurological symptoms after COVID-19 meet diagnostic criteria for somatic symptom disorder (SSD), a condition where psychological distress can amplify physical symptoms. Researchers evaluated 50 patients and found that about two-thirds met SSD criteria, though brain imaging and standard neurological tests were mostly normal. Most patients also had fatigue meeting chronic fatigue syndrome criteria, sleep problems, and significant work loss.
