Researchers tested whether a standard exercise test (cardiopulmonary exercise test, or CPET) could reliably measure how ME/CFS affects the body's ability to exercise. They had 51 women with ME/CFS and 10 healthy women do the same exercise test twice, 24 hours apart. The results showed that the measurements were consistent and clearly different between the two groups, suggesting that CPET could be a useful tool for assessing ME/CFS.

Researchers had 82 women complete exercise tests on two consecutive days and measured how much oxygen their bodies could use and how much work they could do. Patients with severe ME/CFS showed the biggest drop in exercise capacity between day one and day two, suggesting their bodies recover more poorly from physical activity.

They found that people with ME/CFS had higher levels of myelin (the protective covering around nerve fibers) and/or iron in specific brain regions, particularly in areas deep in the brain and in white matter tracts. These changes were linked to ME/CFS symptoms like cognitive problems, heart rate variability, breathing patterns, and physical well-being.

When people have both ME/CFS and health anxiety together, they tend to report worse fatigue and more difficulty with physical activities. The researchers tested whether cognitive behavioral therapy (a type of talking therapy) could help reduce health anxiety in ME/CFS patients, and found it produced meaningful improvements.

Researchers found that blood flow to the brain dropped significantly more in ME/CFS patients (24-29%) compared to healthy people (7%), even in patients whose heart rate and blood pressure stayed normal. The more symptoms patients experienced from standing, the greater their blood flow reduction.

They found that GWI veterans had higher levels of autoantibodies—proteins the immune system makes that mistakenly attack the brain and nervous system—compared to all three control groups. This suggests that a blood test measuring these autoantibodies could potentially help doctors diagnose Gulf War Illness objectively.

Researchers tested whether cyclophosphamide, a chemotherapy drug, could help people with ME/CFS based on observations that some patients improved after cancer treatment. Forty patients received six infusions of this drug over six months, with check-ins lasting up to four years. More than half the patients reported meaningful improvements in fatigue and physical activity, with many staying better even years later.

Understanding these risk factors could help doctors better predict and manage PEM in individual patients.

Teenagers with both ME/CFS and anxiety/depression were more likely to have negative thoughts in general and worry more about their fatigue causing damage or embarrassment. The thinking patterns of both groups together explained about 43% of why some teens developed anxiety or depression alongside their ME/CFS.

The researchers found that those who also have POTS (a condition affecting heart rate and blood pressure when standing) had significantly more digestive issues than those without POTS. The digestive problems remained worse in the POTS group even after accounting for other conditions like chronic fatigue syndrome.

Researchers compared over 80,000 people with a history of childhood maltreatment to 161,000 similar people without this history and found that those with maltreatment exposure had a significantly higher risk of developing several of these conditions. The findings suggest that childhood trauma may be an important risk factor for developing these debilitating syndromes.
