Preliminary determination of the association between symptom expression and urinary metabolites in subjects with chronic fatigue syndrome.

Researchers found that ME/CFS patients have a distinctive substance in their urine called CFSUM1 that is rarely found in healthy people. This study examined whether certain chemicals in urine could be linked to the different symptoms ME/CFS patients experience, such as fatigue, cognitive problems, muscle pain, and digestive issues. They discovered that two specific urine metabolites—CFSUM1 and beta-alanine—were strongly associated with symptom severity, suggesting these could eventually be used as biological markers for diagnosing ME/CFS.

Viral serologies in patients with chronic fatigue and chronic fatigue syndrome.

Researchers tested 548 people with chronic fatigue to see if they had unusually high levels of antibodies to 13 common viruses, since ME/CFS often starts suddenly after a viral illness. They compared antibody levels in patients with chronic fatigue to healthy controls and found no consistent differences between the groups, suggesting these particular virus tests are not helpful in diagnosing or understanding chronic fatigue.

Postinfectious chronic fatigue: a distinct syndrome?

Researchers studied 717 people with chronic fatigue to see if those whose illness started after a viral infection were different from those whose illness began differently. They found that people with post-viral onset reported more flu-like symptoms (fever, chills, sore throat, swollen lymph nodes) and had worse day-to-day functioning, but blood tests and other markers of inflammation or ongoing viral infection did not show clear differences between the two groups.

Psychosocial risk factors for chronic fatigue and chronic fatigue syndrome following presumed viral illness: a case-control study.

This study looked at 64 people who either developed lasting fatigue or didn't after a viral illness, examining their mental health, stress levels, and coping strategies. The researchers found that people who developed chronic fatigue were more likely to have had past mental health problems and used avoidance-based coping methods. Interestingly, those who met the full criteria for ME/CFS had severe fatigue but didn't necessarily have more psychiatric issues than others with chronic fatigue.

While the study suggests a strong link between psychiatric conditions and ME/CFS, it does not explain whether one causes the other.

Hypochondriasis influences quality-of-life outcomes in patients with chronic fatigue.

This study looked at 71 people with chronic fatigue to understand how anxiety about health (hypochondriasis) affects their quality of life. The researchers found that people who worried more about their health and had more physical symptoms reported worse quality of life, especially those diagnosed with ME/CFS. Depression also played a role in this relationship.

Cognitive functioning in patients with chronic fatigue syndrome.

This study found that people with ME/CFS performed worse than healthy controls on thinking and memory tests. Patients showed slower processing speed, difficulty concentrating, and trouble learning and remembering new information. However, the researchers noted that cognitive problems varied widely between patients, so not everyone with ME/CFS experiences the same level of difficulty.

Descriptive epidemiology of chronic fatigue syndrome based on a nationwide survey in Japan.

Researchers surveyed hospitals across Japan in 1992 to understand how many people have ME/CFS and how it spreads. They found that about 0.85 cases per 100,000 people were diagnosed that year, with slightly more women affected than men. The number of cases appeared to be growing from 1992 to 1993, and a small percentage of cases started after an infection.

Researchers measured body temperature and melatonin (a sleep hormone) rhythms in 20 ME/CFS patients and 17 healthy people. They found that in healthy people, body temperature and melatonin rhythms are normally synchronized, but in ME/CFS patients, these two rhythms were disconnected from each other—like two clocks running at different times.

Chronic fatigue and minor psychiatric morbidity after viral meningitis: a controlled study.

Researchers followed 83 patients who had viral meningitis and compared them to 76 people who had other viral infections. They found that about 12.6% of all participants developed chronic fatigue after their illness—higher than expected. Interestingly, having meningitis specifically was not a stronger risk factor than other viral infections; instead, people who took longer to recover and those with a history of psychiatric illness were more likely to develop chronic fatigue.

Metabolic and cardiovascular effects of a progressive exercise test in patients with chronic fatigue syndrome.

Researchers tested how well women with ME/CFS could exercise compared to healthy controls by having them walk on a treadmill until exhaustion. Women with ME/CFS had lower aerobic fitness levels than healthy controls, but their hearts and lungs appeared to work normally. Importantly, the maximal exercise test did not cause a major flare-up of fatigue or other ME/CFS symptoms.

Researchers found that people with ME/CFS had much greater difficulty with physical and mental activities than the general population and most other disease groups—similar in severity only to depression, though in different ways.

Researchers developed a test to measure fatigue by checking how quickly a person's muscle strength drops during repeated effort. They tested this in people with multiple sclerosis (MS), people with ME/CFS, and healthy volunteers. People with MS showed more muscle fatigue than those with ME/CFS or no illness, especially if their MS affected movement control.

This 1996 study examined whether high-dose vitamin C infusions, with or without a hormone called DHEA added, could help treat ME/CFS. Researchers treated 313 patients with chronic pneumonia-like illness that matched ME/CFS symptoms, often combined with antibiotics. The authors found that the combination of two types of vitamin C infusions worked better than one type alone, suggesting that low levels of certain body hormones might play a role in ME/CFS.

Cognitive idiosyncrasies among children with the chronic fatigue syndrome: anomalies in self-reported activity levels.

This study looked at whether children with ME/CFS and their parents perceive their activity levels differently than healthy children do. Researchers used activity monitors (objective measurement) and asked children and parents to estimate activity (subjective measurement) over 3 days. They found that children with ME/CFS and their parents underestimated how much activity was actually happening, and there was a bigger gap between what they expected to do in the future versus what they wanted to do.

Prognosis in chronic fatigue syndrome: a prospective study on the natural course.

This study followed 246 ME/CFS patients for 18 months to see how many improved on their own without treatment. Only 3% fully recovered and 17% reported some improvement. The study found that patients who felt they had some control over their symptoms, had less severe fatigue, had been sick for a shorter time, and didn't focus heavily on physical causes were more likely to improve.

This study compared how the nervous system controls heart rate in people with ME/CFS versus healthy volunteers during and after treadmill walking. Researchers found that people with ME/CFS had weaker parasympathetic (rest-and-recover) nervous system activity during walking, and this activity dropped even further after exercise stopped. This abnormality might help explain why ME/CFS patients feel worse after physical activity.

Memory, attention, and executive function in chronic fatigue syndrome.

This study compared the thinking abilities of 20 people with ME/CFS to 20 healthy controls using computerized tests and word tasks. Researchers found that ME/CFS patients had difficulty with memory tasks that required effort and attention, especially when they had to actively retrieve or organize information themselves. However, their problems were different from those seen in dementia or stroke-related memory loss, and they performed normally on simpler memory tests.

This study examined a specific brain wave pattern during sleep called K-alpha activity and compared it to periodic leg movements during sleep. Researchers found that patients with K-alpha activity were predominantly women, younger, and more likely to have fibromyalgia and chronic fatigue syndrome, while also experiencing unrefreshing sleep, stomach problems, and muscle pain.
