I want to understand PEM

Post-exertional malaise is at the heart of ME/CFS. Understanding it changes everything.

Guidance

  • PEM is not ordinary tiredness. It's a measurable worsening of symptoms triggered by activity that would be trivial for a healthy person.
  • Onset is often delayed — symptoms may peak 24–72 hours after the triggering activity, making the connection hard to spot at first.
  • The response is disproportionate: a short conversation, a trip to the shops, or even a shower can trigger days of inability to function.
  • Two-day cardiopulmonary exercise testing (CPET) has confirmed that people with ME/CFS show objective declines in physical capacity after exertion — this is not deconditioning.
